This packet of surveys are used to evaluate the effectiveness of and satisfaction with hospice and palliative care service delivery to patients, family members and care partners. They have been designed to capture helpful and timely feedback from those you serve and can assist with outcomes and quality measures. The purpose of each survey and suggested method for distribution is also provided. These surveys are not designed to be used as research tools. The surveys can be adapted to the needs of your organization / agency . The following surveys are included in this packet:

Referring Physician Survey- to evaluate service delivery to physicians referring patients to hospice programs

Nursing Facility Survey – to evaluate service delivery to contractual and referring long term care facilities

Nursing Facility Business Office Survey – to service delivery to all contractual and referring nursing facility business offices

Discharge Planner/Social Worker Survey- to evaluate service delivery to contractual and referring facility discharge planners/social workers

Bereavement Survey – to evaluate hospice bereavement service delivery; intended to be sent at the close of bereavement care or 13 months post death

Assisted Living Residence Survey – to evaluate service delivery to assisted living residences
